Child Name(s)*	A comma-separated list of the names of children who will attend the VBS. Please include your child's last name if it is different than your own last name.
Child Age(s), K-12 years*	A comma-separated list of the ages of the children who will attend VBS.
Child grade(s) in school this fall*	Please enter the school grade that your child will be entering this fall, which will help us place your child in the right class in our VBS. For example, if you child is entering sixth grade, you can put 6 or "6th grade." If you have multiple children, you can say "6, 4, 1" in the same order that the children's names are listed above and we will be able to track the information.
Child's Allergies	We need to know about food allergies. If your child has any, please include whether you will bring an epi-pen for them. You may include medication allergies as well, but we do not administer non-emergency medication (epi-pen would be an exception in a life-threatening emergency).
